  • Use the Responsive Declaration to respond to the orders requested in the Request for Order.

  • Use the the Spousal or Domestic Partner Support Declaration Attachment form to provide information on the factors the court must consider in ruling on your request.

  • Use the Income and Expense Declaration to disclose your income and monthly expenses. If you receive income from wages, retirement or cash benefits, attach verification (pay stubs or other written verification) of your income for the past two months. If you receive self-employment income, attach profit and loss statements for the last two years.

  • All Responsive Declaration forms and documents submitted in support must be served to the other party or parties in your case before your hearing.   • Use the Attached Declaration to provide facts in support of your request. 

  • Use the Notice of Intent to Lodge Documents to submit supporting exhibits, such as photographs, emails, text messages, police reports and other records. Please note that while a copy of Notice of Intent to Lodge Documents and your attached exhibits should be served to the other parties with your other Request for Order documents, the court will not accept its copy of the Notice of Intent to Lodge Documents and attached exhibits until 10 days or less before your scheduled hearing.

  • Use the Interpreter Request/Cancellation Form to request a court-assigned interpreter for your hearing.
